What is PMDD?
Premenstrual dysphoric disorder (PMDD) is a severe form of premenstrual syndrome (PMS) that affects mood and emotional functioning in the days or weeks leading up to menstruation.
Unlike typical PMS, PMDD produces intense psychological symptoms that significantly impair a person’s ability to function day to day.
PMDD is classified as a depressive disorder in the DSM-5. It’s neurobiological in nature, meaning it involves the brain’s response to hormonal fluctuations, not abnormal hormone levels themselves. Symptoms typically emerge one to two weeks before menstruation and resolve within a few days of the period beginning.
Approximately three to eight percent of people who menstruate are affected by PMDD. It is not rare, but it is consistently underdiagnosed and undertreated.

How is PMDD Diagnosed?

The standard approach involves tracking symptoms across at least two menstrual cycles to confirm that they emerge in the luteal phase and resolve after menstruation begins. To meet the diagnostic criteria, a person must experience at least five symptoms during the luteal phase (with at least one being a core mood symptom such as depressed mood, mood swings, irritability, or anxiety) severe enough to interfere with daily functioning.
